What the trials actually reported

A 2003 placebo-controlled trial gave 244 adults with functional dyspepsia 1,920 mg of artichoke leaf extract a day for six weeks and found tolerability comparable to placebo overall — the trial didn't flag diarrhea as a distinguishing side effect between groups. A systematic review pooling three cholesterol trials (262 adults) described adverse events across those studies as mild, transient and infrequent, without naming diarrhea specifically. Neither source supports "artichoke extract causes diarrhea" as a documented finding.

The bowel-pattern finding, and its real limits

A subset analysis of a 2-month, open-label study identified 208 adults who had both dyspepsia and IBS symptoms and tracked their self-reported "usual bowel pattern" before and after taking 320 or 640 mg of artichoke leaf extract a day. It found a significant shift away from "alternating constipation/diarrhea" toward "normal," alongside a drop in reported IBS incidence. That's a real, specific result — but it came from an uncontrolled study with no placebo group, so natural symptom fluctuation, the act of being monitored, or expectation effects could all be contributing to what was reported. A separate 6-week post-marketing surveillance study in a similar IBS subgroup reported comparable symptom improvement, also without a placebo comparison. Neither study measured stool frequency or consistency with an objective tool — both relied on self-report.

What that means for "does it make you poop"

Nothing in the research reviewed here supports artichoke extract as a laxative, and nothing supports it causing diarrhea either. The bowel-pattern finding above is about people who started with irregular bowel habits (constipation alternating with diarrhea) moving toward "normal" — not about a healthy digestive system being pushed in either direction. Whether that translates to someone without IBS or dyspepsia symptoms hasn't been tested.
