L-citrulline and medications: what to check
L-citrulline is not a blood thinner and hasn't been tested against any specific medication. The clearest reason for caution is the overlap between citrulline's effect on nitric oxide and the mechanism targeted by PDE-5 inhibitors, prescription nitrates, and blood-pressure medication.
L-citrulline is not a blood thinner, and we didn't find a trial testing it against any specific medication. The one interaction with real pharmacology behind it is the overlap between citrulline's effect on nitric oxide and the mechanism used by PDE-5 inhibitors and prescription nitrate medications.
Is L-citrulline a blood thinner?
No. A blood thinner changes how blood clots, through mechanisms like inhibiting clotting factors or platelet function. L-citrulline doesn't work that way — its documented mechanism is raising plasma arginine, the precursor for nitric oxide, which affects blood-vessel dilation rather than clotting. We haven't found a trial testing L-citrulline against warfarin or another anticoagulant specifically.
PDE-5 inhibitors and prescription nitrates
This is the interaction with the clearest pharmacology behind it, even though it hasn't been tested for L-citrulline directly. A 2023 clinical review describes a well-documented, sometimes dangerous interaction between PDE-5 inhibitors (such as sildenafil or tadalafil) and prescription organic nitrate medications (such as nitroglycerin): combining them can cause a significant drop in blood pressure, because both act on the same nitric-oxide pathway. L-citrulline raises the body's own arginine and nitric-oxide activity through a related mechanism, and while no trial has tested combining a citrulline supplement with either drug class, the shared pathway is a reasonable basis for caution. Ask a doctor before combining L-citrulline with a PDE-5 inhibitor or a prescription nitrate medication.
Blood pressure medications generally
L-citrulline has lowered blood pressure in trials of adults who already have elevated blood pressure — see L-citrulline benefits: what the research does and doesn't show for the specifics. We haven't found a trial testing L-citrulline alongside a prescription blood-pressure medication, but given that documented effect, ask your doctor before adding L-citrulline if you're being treated for high blood pressure.
Other nitric-oxide-related supplements
Some products combine L-citrulline with L-arginine, beetroot extract, or other dietary nitrate sources that work through the same or an overlapping pathway. We haven't found a trial testing whether stacking several of these together changes the effect, or the safety profile, compared with any one of them alone.
Summary table
| Medication | What the research shows | What to ask about |
|---|---|---|
| Warfarin / other blood thinners | No trial found; L-citrulline isn't a blood thinner and works through a different mechanism | Mention any new supplement, as routine practice |
| PDE-5 inhibitors (sildenafil, tadalafil) / prescription nitrates | No trial found for L-citrulline specifically; a well-documented interaction exists between the drugs themselves via the nitric-oxide pathway | Ask before combining, given the shared mechanism |
| Blood pressure medications generally | No trial found; L-citrulline has lowered blood pressure on its own in people who already have it elevated | Ask your doctor, given the overlap |
| Other nitric-oxide supplements (L-arginine, beetroot) | No trial found testing the combination | Mention any supplement stack to your doctor |
Questions people ask
Can I take L-citrulline with Viagra or Cialis?
We haven't found a trial testing this combination. Given that PDE-5 inhibitors and citrulline both act on the nitric-oxide pathway, ask a doctor before combining them.
Does L-citrulline interact with blood pressure medication?
No trial has tested this directly. Given citrulline's documented blood-pressure-lowering effect in people who already have elevated blood pressure, ask your doctor before combining it with a blood-pressure medication.
Is L-citrulline a blood thinner?
No. Blood thinners work through clotting mechanisms; L-citrulline's documented mechanism is the arginine/nitric-oxide pathway, which is unrelated.
Can I take L-citrulline with beetroot or L-arginine?
We haven't found a trial testing that stack. Both work on an overlapping nitric-oxide pathway, so combining them is untested territory rather than a confirmed problem.

- Phosphodiesterase Type 5 Inhibitors and Oral Nitrates in Male Patients with Ischemic Heart Disease PDE-5 inhibitors (like sildenafil) and prescription nitrate medications (like nitroglycerin) can interact dangerously through a shared nitric-oxide pathway; this review is about the prescription drugs, not dietary nitrate from beetroot, but the shared mechanism is the reason doctors are often asked about combining nitrate-rich supplements with these medications.
- Dose-ranging effects of citrulline administration on plasma amino acids and hormonal patterns in healthy subjects: the Citrudose pharmacokinetic study A 2008 dose-escalation study gave 8 healthy men single oral doses of citrulline from 2 g up to 15 g and found no side effects at any dose, but at the two highest doses the expected rise in blood arginine was smaller than predicted — the researchers think the kidneys' conversion of citrulline into arginine becomes saturated at higher doses.
- Influence of L-citrulline and watermelon supplementation on vascular function and exercise performance A 2017 review by a research group active in this area found that a single dose of citrulline reliably raises blood arginine but has an inconsistent effect on blood flow itself, while several weeks of daily use is linked to lower blood pressure specifically in people who already have elevated blood pressure, not in people with normal blood pressure.
