Berberine and milk thistle

Berberine and milk thistle are commonly stacked together, but no trial has isolated the two-ingredient combination — the closest research tested a four-ingredient formula that can't be attributed to either ingredient alone.

By Electric Tiger Editorial

No trial has tested berberine and milk thistle as a two-ingredient combination. The closest research is a study of a four-ingredient nutraceutical that included both, plus red yeast rice and guggul — which means its results can't be attributed to berberine and milk thistle on their own.

What is berberine and milk thistle?

They're two separate supplement ingredients often stacked together: berberine, an alkaloid studied mainly for blood sugar and lipid markers, and milk thistle, an herbal extract whose main compound (silymarin) is studied mainly for liver-related markers. Combination products pairing the two exist, but pairing them in a product isn't the same as testing the pairing in a trial.

The closest research that exists

A 2023 randomized, placebo-controlled trial gave 80 adults with mildly elevated LDL cholesterol one pill a day of a combined formula — 400 mg of Berberis aristata extract (providing 340 mg of berberine), 98 mg of red yeast rice extract, 87.5 mg of milk thistle extract (providing 70 mg of silymarin), and 50 mg of guggul extract — or a placebo, for 8 weeks. Because the formula combined four active ingredients, including red yeast rice — a source of monacolins, the same class of compound found in statin medications — any result can't be separated into what berberine did, what milk thistle did, or what either contributed on its own. It also used 340 mg of berberine, well above the 73 mg our own product provides, and none of the four ingredients was tested alone in this trial. See berberine and cholesterol: what the research does and doesn't show for what the trial measured and found.

No trial has isolated berberine and milk thistle as a standalone pair, at any dose, for any outcome.

A reason to mention both to a doctor together

Berberine and milk thistle each affect liver enzymes that process medications, through different and only partly overlapping mechanisms. Berberine has a documented, broad interaction profile across several CYP enzymes and transporters; milk thistle's evidence is smaller and inconsistent — one human trial found a genotype-dependent effect on a blood pressure drug's levels, while another found no effect on four major drug-processing enzymes. Taking both supplements at once, especially alongside a prescription medication, is a reason to mention the full combination to a doctor rather than checking each ingredient separately. See berberine and medications and milk thistle and medications for what's documented on each.

Questions people ask

Can you take milk thistle and berberine together?

No trial has tested the two-ingredient combination directly. The nearest research tested a four-ingredient formula that included both alongside red yeast rice and guggul, which makes it impossible to credit berberine and milk thistle specifically for the result. If you take a prescription medication, mention both supplements to a doctor.
