Does inulin help with diabetes? What the research does and doesn't show
A 2019 dose-response meta-analysis of 33 trials in people with prediabetes or type 2 diabetes found glucose, HbA1c and insulin-resistance markers smaller with inulin-type fructans, most consistently at 10 g/day for 6+ weeks. No comparable trial exists for PCOS.
Most of the trial evidence on inulin and blood sugar comes from people with prediabetes or type 2 diabetes, not a general population. A 2019 dose-response meta-analysis of 33 randomized trials found fasting glucose, HbA1c and insulin resistance markers came out smaller with inulin-type fructans than placebo — an effect linked to a daily dose of 10 g or more, taken for 6 weeks or longer.
What the evidence doesn't support
It doesn't support inulin as a treatment for type 2 diabetes on its own, a replacement for glucose-lowering medication, or a fix for a single blood sugar reading. It also doesn't extend to PCOS-related insulin resistance specifically — the trials behind the numbers below enrolled people with prediabetes or type 2 diabetes; we found no comparable randomized trial testing inulin in women with PCOS, so that's a real gap, not an oversight.
What the trials found
A 2019 systematic review and dose-response meta-analysis in the Journal of Translational Medicine pooled 33 randomized controlled trials, 1,346 participants total, testing inulin-type fructans (ITF) against placebo or control. In the subgroup with prediabetes or type 2 diabetes specifically, fasting blood glucose came out about 0.60 mmol/L smaller with ITF, HbA1c about 0.58 percentage points smaller, fasting insulin about 1.75 µU/mL smaller, and HOMA-IR (a marker of insulin resistance) about 0.69 points smaller — all statistically significant differences versus placebo. The dose-response modeling in that review pointed to 10 g/day for 6 weeks or longer as the amount and duration associated with the clearest separation from placebo; shorter or smaller-dose trials showed less consistent results. A separate, smaller 2020 meta-analysis (9 trials, 661 people with type 2 diabetes) reported similar directional findings for fasting glucose, HOMA-IR and HbA1c, with a stronger signal in trials lasting 8 weeks or more.
Both reviews pooled trials that varied in inulin source, chain length and background diet, and neither review reports results broken out by sex, medication use, or baseline glycemic control in a way that lets a single number apply to any one person's situation.

Inulin and diabetes
The largest evidence pool — 33 randomized trials in people with prediabetes or type 2 diabetes — found fasting glucose, HbA1c and insulin resistance markers came out smaller with inulin-type fructans than placebo, most consistently at 10 g/day or more for 6 weeks or longer. That's evidence about a marker moving in trials, not a substitute for diabetes management.
Inulin powder for diabetes
The trials behind the finding above used powdered inulin-type fructans mixed into food or drink, generally in the 10 g/day range. A powder sold as a general fiber supplement isn't the same thing as a diabetes treatment, and no trial has tested inulin against, or alongside, glucose-lowering medication.
Inulin for PCOS
We couldn't find a published randomized trial testing inulin specifically in women with PCOS or PCOS-related insulin resistance. The insulin-resistance evidence above comes from people with prediabetes or type 2 diabetes, a different population, so it doesn't transfer directly to PCOS.
When to talk to a doctor
Diagnosed diabetes, prediabetes or PCOS are conditions to manage with a doctor, including any decisions about diet, monitoring and medication. Inulin is not a treatment for any of them, and the trial results above describe group averages in supervised studies, not what happens for an individual person. Anyone on insulin or a glucose-lowering medication should talk to a doctor before adding a fiber supplement, since changes in gut fermentation can affect how food and medication are absorbed.
