Research review
Does papaya enzyme help with acne or rosacea? What the research does and doesn't show
No trial has tested plain papain, alone, against a placebo for acne, and no trial at all was found for rosacea. One open pilot study found a reduction in acne severity using a topical gel that combined 2% papain with two retinoid ingredients, with no placebo group and no way to isolate papain's own contribution.
The evidence doesn't support papain by itself as a treatment for acne, and there's no trial at all for rosacea. One open pilot study found a topical gel containing 2% papain, alongside two retinoid ingredients, was associated with a reduction in acne severity over 12 weeks — but with no placebo group and three active ingredients in the same product, the result can't be credited to papain specifically.
What the evidence doesn't support
It doesn't support papain, used alone, as an acne treatment — no trial tested plain papain by itself against a placebo. It doesn't support any papaya-derived product for rosacea at all; no trial testing papain, papaya extract, or any papaya product in people with rosacea was found. And it doesn't support an oral papain supplement doing anything for skin — every study below tested a topical product applied directly to skin, not a swallowed capsule.
What the trials found
A 2015 open, multicenter pilot study gave 98 people with mild-to-moderate comedonal-papular facial acne a gel combining three ingredients — a synthetic retinoid (hydroxypinacolone retinoate), retinol, and 2% papain — applied once daily for 12 weeks. Using a standard acne-severity scale and a lesion count, the study measured a 41% average reduction in severity score and a similar reduction in total lesion count by the end of the study; about 15% of participants had mild-to-moderate local irritation (dryness, peeling, redness, burning), and none stopped treatment because of it. This study had no placebo or untreated comparison group — every participant used the same active product — and papain was one of three active ingredients in the formula, alongside two retinoids that have their own separate research history for acne. Nothing in this design isolates what papain itself contributed, if anything, and the study's own authors call for a controlled trial to confirm the result.
No trial testing papain, or any papaya-derived product, in people with rosacea was found at all — not a positive result, not a null one, nothing. That's a real gap in the research, not a hidden negative finding.
Questions people ask
Does papaya enzyme help with acne?
One open pilot study found a reduction in acne severity in people using a gel that combined 2% papain with two retinoid ingredients over 12 weeks, but with no placebo group and three active ingredients in the product, the result can't be attributed to papain on its own. No trial has tested plain papain by itself against a placebo for acne.
Is papaya enzyme good for acne?
See above — the closest available evidence used papain as one of three ingredients in a topical gel, not by itself, in a study with no placebo comparison. That's a starting point for more research, not a demonstrated benefit for papain alone.
Does papaya enzyme help with rosacea?
No trial testing papain or any papaya product in people with rosacea was found. This is an area with no research at all, in either direction, rather than research that found no effect.
Is a papaya enzyme peel good for skin?
A "papaya enzyme peel" is a topical exfoliant, and the acne-gel study above used 2% papain alongside two other active ingredients, not a standalone peel product — no trial tested a papain-only peel against a placebo for any skin outcome. Papaya enzyme cream, lotion and spray: what these products are covers what these topical products are, separate from what they've been shown to do.
When to talk to a doctor
Persistent or severe acne, and rosacea specifically, are conditions a dermatologist can properly diagnose and treat — papaya enzyme products, topical or oral, are not a treatment for either. If a topical product causes ongoing irritation, redness or a rash, stop using it and talk to a doctor, particularly if you have sensitive skin or a known latex allergy, which is linked to a documented cross-reaction with papaya.
