Is this fruiting body or mycelium?

Both. The panel reads Mushroom Fruiting Body & Mycelium Powder, and it does not state the split between them, what the mycelium was grown on, or how the polysaccharides were measured. We don't know the ratio, so we don't describe it as fruiting body alone or as mycelium-free.

Is it an extract?

No. It is a powder, 1,000 mg per serving of two capsules. Many lion's mane trials use extracts, such as a 10:1 fruiting-body extract, or mycelium standardized to a specific compound. Those are different materials from this one, and their doses don't carry over to ours.

What does 40% polysaccharides mean?

The label states 400 mg of the 1,000 mg is polysaccharides. Polysaccharides is a broader measurement than beta-glucans, and can include starch, which matters most if the mycelium was grown on grain. The label prints no beta-glucan, triterpene, hericenone or erinacine amount.
